About this event

The Heart Foundation invites clinicians working in regional areas in and surrounding Echuca to an exciting and interactive workshop on heart failure self management and the promotion of a safe discharge.

Heart failure expert clinicians from Melbourne will run this workshop and will focus on ways of reducing readmissions and how to best educate your patients in heart failure management.

Participants will be introduced to the 5 steps to a safe discharge checklist, a tool to guide clinicians in key priority areas on discharge. There will also an opportunity to implement a local quality improvement project in your workplace with support from the Heart Foundation.

Local health services that are available to support your patients who are living with heart failure will also be showcased at the workshop.

Please join us for this opportunity to focus, learn and collaborate in supporting patients living in regional Victoria with a heart failure diagnosis.
